Community Plus is an initiative which has the potential to make a real difference to residents.
Community Plus provides short term, community-based interventions which supports residents to reduce isolation and prevent or delay demand on social and health care services – it promotes people’s well-being and independence. The type of support Community Plus provide is broad and varied.
Community Plus works with all age groups and anyone can make an introduction for themselves, or for someone with their consent to the service. After making your introduction a Community Coordinator will make contact and arrange a meeting that suits you. Community Plus has built strong links with community organisations who they work alongside to provide advice, information and guided signposting.
Community Plus looks to connect people to their local community. Support independent, promotes physical, emotional, mental well-being and address isolation.
